Southside Youth
Outcome: Youth develop social and life skills necessary for successful adulthood.
Indicator: Youth demonstrate respect for authority and each other as indicated by 10 or fewer ejections (for total population) from the program due to behavior. 5 ejections
Outcome: Youth Club participants demonstrate character as indicated by participation in 2 or more group volunteer projects.
Indicator: New for 2009, 1 Project in Fall 2008
Outcome: Youth attend school and have passing grades.*
Indicator: Youth have passing grades as indicated on quarterly report card.
Indicator: Youth attend school 95% of the time as indicated on quarterly report card.
* Changes in the laws regarding privacy around public schools have made it difficult for us to enforce our “no school/no Wesley Center” rule and required us to change our indicators around school success. At this point, the only way we have of knowing if students are attending school and getting passing grades is to ask the children for their report cards at the end of each quarter.
Outcome: Youth have skills to help them develop into productive adults.
Indicator: Wesley Weightlifters are successful in school as indicated by passing grades in all subjects. 100% (40/40)
Indicator: Demonstrate knowledge of self discipline as indicated by qualifying for National or International Competitions. 93% (37/40)
Individual and Family Services
Outcome: Individuals and families with limited resources have assistance meeting basic shelter-related needs when in crisis.
Indicator: Eligible persons facing foreclosure, eviction, or utility disconnection have necessary assistance to halt event and assure continued housing. 100% (298/298)

Outcome: People have a way to meet basic needs for food and clothing.

Senior Nutrition
Outcome: Homebound seniors requiring nutritious meals in order to stay healthy and avoid nursing home placement have access to daily meals.

Outcome: Seniors requiring nutritious meals and regular social interaction in order to stay healthy and remain living independently in their own homes have access to daily meals and socialization.
Indicator: Clients report enjoying a better quality of life because of volunteering, socialization and additional services offered at meal sites. 91% (20/22 surveyed)

In-Home and Professional Medical Services
Outcome: Homebound elderly and those who have trouble getting to medical appointments due to infirmity or transportation issues have in-home medically-related services and support to assure their ability to continue to live at home.
Indicator: Low income seniors maintained or gained weight while receiving supplements. 80% (52/65)
Indicator: Medically fragile seniors avoid premature nursing home placement as a result of medical services and support. 93% (103/111)
Outcome: Homebound elderly have in-home services and support to assure their ability to continue to live at home.

Retired and Senior Volunteer Program – RSVP
Outcome: Volunteers have impact on community issues.
Indicator: Eligible individuals requesting meals receive hot meals each day of eligibility either through home delivery or congregate meal sites. 95% (707/747)
Indicator: Increase in EITC eligible persons served by the tax preparation program receiving EITC as a result of volunteer tax preparation, optimizing dollars returned to the community. (174 filings in 2007) 20% Increase (209-174) / 174)
